A 2025 systematic review in The Lancet Public Health found that walking about 7,000 steps per day was associated with a 47% lower risk of death from any cause compared with about 2,000 steps, along with large reductions in cardiovascular disease, dementia and depression risk. At 10,000 steps the reduction was roughly 48% — one extra point for 3,000 extra steps.
The honest version of the answer: step count matters enormously at the low end, matters less as you climb, and 10,000 is a reasonable target rather than a medical threshold.
Where the 10,000-step figure actually came from
The 10,000-step goal was a product name before it was ever a health recommendation. In 1965, the Japanese clock and instrument company Yamasa Tokei Keiki launched a consumer pedometer called the Manpo-kei, which translates roughly to "10,000 steps meter." The launch followed the 1964 Tokyo Olympics, at a moment when public interest in fitness in Japan was unusually high. The number was chosen because it was round, memorable and easy to print on a package — and because the Japanese character for 10,000 (万) loosely resembles a walking figure.
No clinical trial established 10,000 steps as a threshold before the pedometer was named. Harvard epidemiologist I-Min Lee, who went on to lead one of the largest studies of step counts and mortality, has said directly that no studies had examined the 10,000 figure at the time it entered circulation. Walking-behaviour researcher Catrine Tudor-Locke has traced the slogan's global spread back to that 1965 product launch. What happened next is that a marketing number was adopted by health campaigns, then by fitness trackers as a default daily goal, and eventually came to be treated as though it had a clinical origin.
This history matters for a practical reason rather than a trivia reason. Treating 10,000 as a pass/fail line makes 9,000 steps feel like a failed day, which it is not. Understanding that the number was picked for a package makes it easier to treat it as a useful ceiling rather than a minimum standard.
The benefits of walking, broken down by outcome
Longevity and risk of early death
The relationship between daily steps and mortality is the most heavily studied question in this field, and the findings are consistent across large populations. The 2025 Lancet Public Health review — a dose-response analysis pooling 57 studies from more than ten countries — found that 7,000 steps per day was associated with a 47% lower risk of all-cause mortality compared with 2,000 steps per day.
A 2019 study in JAMA Internal Medicine, which followed 16,741 women with a mean age of 72, found that women averaging roughly 4,400 steps per day had a 41% lower mortality rate over the follow-up period than women averaging roughly 2,700 steps per day.
The most useful takeaway from that study is not the headline number but the shape of the curve. Mortality risk fell steadily as daily steps increased and then levelled off at roughly 7,500 steps per day. Moving from a very low step count to a moderate one produced far more benefit than moving from a moderate step count to a high one.
Cardiovascular health
Walking affects heart disease risk at step counts most people can reach without restructuring their day. The 2025 review found that 7,000 steps per day was associated with a 25% lower risk of developing cardiovascular disease and a 47% lower risk of dying from it, both compared with 2,000 steps per day.
Those two figures are worth separating. The 25% reduction refers to whether cardiovascular disease develops at all. The 47% refers to mortality among the outcomes tracked — which suggests walking is associated with both lower incidence and better survival, though observational research cannot establish that walking caused either result.
Metabolic health and type 2 diabetes
Daily step count shows a clear association with diabetes risk. The 2025 review found 7,000 steps per day was associated with a 14% lower risk of type 2 diabetes compared with 2,000. Walking improves insulin sensitivity and helps clear glucose from the bloodstream, and short walks after eating have a measurable effect on the size of the blood-sugar spike that follows a meal.
Weight, and an honest caveat about it
Step count is a weaker tool for weight loss than most fitness marketing suggests. The 2025 review examined mortality, cardiovascular disease, cancer, type 2 diabetes, cognitive outcomes, mental health, physical function and falls — and did not include body weight or obesity among its outcomes.
Walking 10,000 steps burns somewhere in the region of 300–500 calories for a typical adult, though the real figure swings widely with body weight, pace and terrain. That is an amount easily offset by a single snack, and appetite tends to rise somewhat in response to increased activity. The metabolic case for walking is strong; the weight-loss case is modest. It is more useful to know that in advance than to abandon the habit after six weeks because the scale has not moved.
Mental health and mood
The link between daily step count and depression risk is stronger than most people expect. The 2025 review found 7,000 steps per day was associated with a 22% lower risk of depressive symptoms compared with 2,000. Walking outdoors adds daylight exposure, which independently supports circadian rhythm and sleep quality — and sleep quality is itself one of the strongest predictors of mood.
The practical implication: walking is one of very few interventions with a low barrier to entry and a documented association with mental health outcomes. No equipment, no membership, no skill to acquire.
Cognitive health and dementia risk
Dementia showed one of the largest associations in the recent research. The 2025 review found 7,000 steps per day was associated with a 38% lower risk of dementia compared with 2,000, with no meaningful statistical heterogeneity between the pooled studies for that outcome.
Worth flagging for anyone whose step-count motivation is currently framed entirely around weight or appearance: the cognitive association is larger in magnitude than the diabetes association, and considerably larger than anything walking does for body weight.
Joint health and fall risk
Walking does not wear out knees, and there is direct evidence pointing the other way. A 2022 study in Arthritis & Rheumatology examined 1,212 adults aged 50 and older who already had knee osteoarthritis, drawn from the Osteoarthritis Initiative cohort. Among those without regular knee pain at baseline, 26% of the people who walked for exercise developed new frequent knee pain over eight years, against 37% of those who did not — roughly 40% lower odds. The researchers also found preliminary evidence that walking may slow structural damage inside the joint.
Fall risk also declines. The 2025 review found 7,000 steps per day was associated with a 28% lower risk of falls compared with 2,000 — which matters more with age than almost any other outcome on this list.
So how many steps do you actually need?
The evidence points to a threshold below 10,000 for most outcomes, and to a threshold that shifts with age.
A 2022 meta-analysis in The Lancet Public Health pooled 15 international cohort studies covering 47,471 adults and found that mortality risk declined progressively with more steps until roughly 6,000–8,000 steps per day for adults aged 60 and older, and until roughly 8,000–10,000 steps per day for adults younger than 60. Adults in the highest step-count quartile had a 40–53% lower mortality risk than adults in the lowest.
The 2025 review found inflection points across its various outcomes at roughly 5,000–7,000 steps per day, and concluded that 7,000 steps is associated with clinically meaningful improvements while remaining a more realistic target for many people than 10,000. That same review found that even about 4,000 steps per day was linked to better outcomes than about 2,000 — the most important sentence in this article for anyone currently sedentary.
Pace matters less than total volume. The 2019 JAMA Internal Medicine study measured several stepping-intensity metrics, including peak one-minute and peak 30-minute cadence, and found that once total daily steps were accounted for, the associations between intensity and mortality weakened substantially and were mostly no longer statistically significant. Walking faster is not required for the longevity benefit. Walking more is what the data supports.
One caution applies to all of this. These are observational cohort studies, not randomised trials, so they establish association rather than causation. People who walk more may simply be healthier to begin with, and researchers adjust for that as best they can without eliminating the possibility entirely.
How to reach 10,000 steps without adding a workout
Most people who hit a high daily step count are not walking for 90 minutes. They are accumulating steps in fragments across a day that was already happening. If you are tracking those fragments on a wrist device, it is worth knowing how accurate your tracker actually is — most undercount rather than overcount.
Take calls on your feet. A 30-minute phone call walked rather than sat is roughly 3,000 steps — close to a third of the daily target at zero additional time cost.
Walk 10–15 minutes after meals. Three short post-meal walks add roughly 3,000–4,500 steps and produce a measurable reduction in the blood-sugar spike that follows eating. This one does double duty.
Park at the far end of the lot, every time. The rule works because it removes the decision. A consistent extra 200–400 steps per errand compounds across a week.
Use the long route deliberately. The bathroom on the other floor, the farther coffee shop, the entrance on the opposite side of the building — almost no time cost, several hundred steps per instance.
Break up long sitting blocks with a five-minute walk every hour. Eight of those across a workday is roughly 4,000 steps, and interrupting prolonged sitting has metabolic benefits independent of the step total.
Front-load the morning. People who reach 3,000–4,000 steps before mid-morning hit their daily target far more often than people who leave the whole count until evening. A large deficit at 6pm is what usually turns into a skipped day.
Staying consistent is the actual problem
None of the research above is difficult to act on for a week. The difficulty is week five, when the novelty has faded, the weather is bad, and the number stops feeling meaningful. Step count is unusual among health habits in that the barrier is almost never knowledge or capability — it is almost always attention.
Two things reliably help.
Set a floor, not a goal. A minimum you hit every single day builds a stronger habit than an ambitious target you hit twice a week. Six thousand every day beats twelve thousand on Tuesdays.

Do treadmill steps count the same as outdoor steps?
For step-count purposes, yes — wrist- and hip-worn trackers detect the motion of walking rather than distance travelled over ground. Holding the treadmill handrails can cause a wrist-worn tracker to undercount, because the device relies on arm swing to register steps. Outdoor walking involves more variation in terrain and slight balance adjustments, so it engages stabilising muscles somewhat more than a flat belt does.
